Certificate Account Instructions

Use the Certificate Account Instruction Form to:

  • Reinvest or distribute funds from a maturing certificate account (please complete the Certificate Account Closeout section below)
  • Withdraw dividends from your certificate account (please complete the Certificate Account Dividends Withdrawal section below)

Since this is a secure form, all your account information is protected.

Please note: If you would like to add a joint owner or beneficiary to your certificate account, please call our Contact Center at 845.463.3011, or visit any of our branches. Additionally, if you are providing instructions for more than one certificate account, you will need to submit the form below for each of those accounts.

IMPORTANT INFORMATION ABOUT THE PROCEDURES FOR OPENING A NEW ACCOUNT

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask you for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
